Functional Capacity Assessments

Assessments and reports detailing functional abilities, and support needs as a result of disability, injury or illness.

These are typically done to support plan reviews due to inadequate funding, in advance of regular plan reviews or as a result of a change in situation/circumstances.

These normally require at least 1-2 in-person visits, along with administering of selected assessment tools, interview with client and/or relevant stakeholders (family/support workers etc) and review of relevant collateral (incident reports, GP/Specialist letters, hospital summaries etc).

Home and Living Assessments

  • Supported Independent Living (SIL) assessments

  • Individualised Living Options (ILO) assessments

  • Specialist Disability Accommodation (SDA) assessments

These assessments can be lengthy and require significant evidence to justify particular recommendations or funding options. A Functional Capacity Assessment is typically completed as part of the process, which helps to inform suitable options.

Home safety and independence review

Review of safety and independence around the home. These can be completed at any time, but typically are useful if an individual has experienced falls, deterioration in their ability or their living situation has changed. It is also common to complete these post-discharge from hospital after an injury or illness.

  • Mobility review

  • Safety aids - rails, threshold ramps etc

  • Mobility equipment - walkers, over-toilet frames etc

About me

I’m Tara Reynolds, an experienced Senior Occupational Therapist who is now practicing in the Hunter Valley area. As I have just opened my books in this area, I’m accepting referrals with minimal wait times. 

I have 15 years experience as an OT, the last 6 of which I have mostly worked with NDIS clients. I have experience across diverse areas, and have worked in hospital, community and private settings.

I am from the Hunter Valley originally, and have worked in Newcastle, the Central Coast, Sydney and Brisbane. I would describe myself as a Mental Health OT, but feel equally comfortable working with clients with intellectual disabilities or neuro-developmental disabilities (including autism), or with physical disabilities. I have plenty of experience working with complex clients who have dual-diagnoses, or clients navigating challenging circumstances including accommodation changes, hospital discharges or AAT appeals.
